How do I request a form?
Use the "Request Digital Form" button to request the creation or editing of a digital form.
What digital form services can I request?
You can request the creation of a new digital form for use on a College website or Portal site, or for other business purposes such as insertion in emails etc. Web Services will evaluate your request, contact you, and decide the best platform to use based on your data needs.
You can also request editing of an existing digital form.
Who can request a form?
Faculty and staff.
